The Pillow Under Her Hips Trick: Science of the Perfect Missionary

The pillow trick is the simplest modification you can make to missionary position — and it has peer-reviewed research backing its effectiveness. Place a pillow under her lower back, and you transform standard missionary into a precision tool for G-spot and cervical stimulation.

The Science: Coital Alignment Technique (CAT)

In 1988, researcher Edward Eichel published studies on the Coital Alignment Technique (CAT) — a modified missionary where the man shifts higher so the base of his penis maintains constant contact with the clitoris. An 8-week clinical study showed significant increases in female orgasm rates, including simultaneous orgasm.

The pillow technique achieves a similar biomechanical effect through a different mechanism: instead of the man moving higher, the woman’s pelvis is tilted upward. The result is the same — the penis naturally angles toward the anterior vaginal wall where the CUV complex concentrates.

How to Do It

  1. Place a firm pillow under her tailbone/lower back. The pillow should elevate her pelvis by approximately 15-20 degrees. A folded towel works if a pillow is too thick.
  2. Enter in standard missionary. You’ll immediately feel a different angle — the penis head naturally presses upward against the anterior wall.
  3. Apply the 5-second pause after full insertion.
  4. Use grinding motions at full depth rather than long in-out strokes. Press and rotate against the anterior wall.
  5. Clench your glutes to maximize pressure transfer.
  6. Hold her hips or thighs to prevent her body from sliding away from you.

Why This Is My Go-To Starting Position

In my standard rotation (Missionary → Cowgirl → Doggy → Prone Bone → Missionary), I always start with pillow missionary because:

  • It’s the most natural starting position — no awkward setup required
  • It allows maximum intimacy (face-to-face, kissing, eye contact) during the critical early phase
  • The pillow modification immediately puts your penis on the right trajectory for CUV stimulation
  • For women who’ve never had a vaginal orgasm, this position has the highest first-time success rate in my experience

Combining with Clitoral Stimulation

The elevated pelvis also makes it easier to reach her clitoris with your hand during penetration — or you can ride slightly higher (true CAT position) so your pubic bone maintains contact with her clitoris throughout each grinding motion.

This simultaneous internal and external stimulation activates the entire CUV complex — the scientific basis for the most intense orgasms.
